Visiting Hours
Visiting hours have been prepared to help provide a safe, therapeutic environment for our patients. In some cases, visiting hours may be limited or expanded in accordance with the physician's treatment recommendations.
The hours are from 9:00 a.m. through 9:00 p.m. The number of visitors in each patient's room should be limited to two persons. Children under 12 years old should wait in the designated waiting areas. The charge nurse on duty may set more restrictive limits as necessary based on the patient's condition.
Gift Shop
Our gift shop is located at the main entrance of the hospital. The gift shop is open Monday through Friday from 9:00 a.m. to 4:00 p.m. The shop is operated by the Memorial Hospital Auxiliary. Profits from the sales are used to support hospital district projects and purchase special equipment to enhance the quality of care our patients receive. In the gift shop you will find many items and gift ideas.
Financial Arrangements
After you are discharged from the hospital, you will receive an itemized statement reflecting daily charges including supplies and support services. If you have any questions about your bill, please call the business office at 758-4865. You will receive a separate bill from the physicians involved in your care and treatment. This includes pathologists who interpret laboratory results, and radiologists who read, interpret and forward results of radiology, ultrasound, CT and MRI exams to your physician.
Insurance
Whether you have individual coverage, Medicare, Medicaid or other forms of insurance, please remember to bring your insurance card and other identification with you each time you come to the hospital.
Most hospital insurance policies do not cover all expenses incurred. In some cases, we may ask the patient to pay the deductible and co-insurance portions before or during their stay.
Some insurance plans require pre-certification prior to admission. Please follow all requirements set forth in your insurance plan and make any arrangements deemed necessary by your insurance company.
Patients who are not insured will be asked to make a deposit before or at the time they are admitted. The balance of the hospital bill can be paid when they are discharged, unless other arrangements have previously been made.
Patient Rights and Responsibilities
The Health Care Professionals at Memorial Hospital recognize that each patient is an individual with unique healthcare needs. We respect the dignity and worth of every person, and continually strive to provide considerate, respectful care focused upon the patient's individual needs. We believe a person's health is affected by environmental and emotional factors, and that being able to communicate concerns with the health care team is important to the healing process as well as being the primary right of the patient. Our organization assists patients in exercising right and informs patients of any responsibilities incumbent upon them in the exercise of those rights.
Patient Rights
We affirm the prerogative of the patient to:
Exercise these rights without regard to sex, culture, economic status, educational or religious background, or the source of payment for care.
Receive consideration for developmental needs and respectful care, care which includes consideration of psychological, spiritual and cultural variables that influence the perceptions of illness.
Receive consideration for developmental needs with activities and treatment to include services to neonates, children, adolescent and geriatric patients.
Know the name of the physician who has primary responsibility for co-coordinating care and the names and professional relationship of other physicians who are treating the patient.
In collaboration with physicians, making decisions involving personal health care including: receive information from the physician about the diagnosis, course of treatment, and results to be expected from the treatment in terms understandable to the patient.Receive as much information about any proposed treatment or procedure as may be needed in order to give informed consent or to refuse that course of treatment. Except in emergencies, this information shall include a description of the procedure or treatment, the significant risks involved in this treatment, alternative courses of treatment or nontreatment and the risks involved in each, as well as the name of the person who will carry out the procedure or treatment.
Receive full consideration of privacy concerning the medical care program. Case discussion, consultation, examination, and treatment are confidential and shall be conducted with discretion. The patient has the right to be advised as to the reason for the presence of any individual.
Receive reasonable responses to any reasonable request for service. leave the hospital even against the advice of his or her physician.
Consult with another doctor for the purpose of getting a second opinion. As required by the case, the hospital will provide evaluation and/or referral service. examine and receive and explanation of the bill regardless of the source of payment.
Know which hospital rules and policies apply to their conduct as a patient. have all patient rights apply to the person who has legal responsibility to make decisions regarding medical care on behalf of the patient.
Patient Repsonsibilities
Each patient has the responsibility to:
Respect the rights of other patients. Follow the procedures, rules and regulations established for the mutual benefit of all patients.
Cooperate in the treatment plan recommended by the physician. This may include following the instructions of nurses and hospital personnel as they carry out the coordinated treatment plan and implement the physician's orders.
Provide to the best of his/her knowledge, accurate and complete information about present complaints, past illnesses, hospitalizations, medications, and other matters relating to his/her health.
Report to the physician and/or nursing staff unexpected changes in his/her condition.
Make known whether he/she clearly comprehends the proposed course of treatment and what is expected of him/her. assure that the financial obligations of his/her health care are fulfilled as promptly as possible.
Patient Self Determination Act
As a patient in Memorial Hospital, you have certain legal rights concerning your medical treatment. Formal policies have been adopted to insure that your rights to make medical treatment decisions will be honored to the extent permitted by law. This hospital has adopted policies relating to the informed consent, implementation of Directives to Physicians under the Texas Natural Death Act and implementation of treatment decisions made by agents appointed by a durable Power of Attorney for Health Care.
